The investigation further stated that the payroll vendor system was upgraded in January and February 2026, which increased the time needed for the facility to review and validate the data, and that the coding issue was not identified before the contracted service submitted the file. The facility requested an appeal to CMS, but CMS denied it and stated that late PBJ data could not be submitted or corrected after the deadline.
