Summary

The facility failed to establish and maintain an infection prevention and control program when Treatment Nurse A and ADON C did not wear a gown while performing wound care on Resident #1, who was on Enhanced Barrier Precautions (EBP). Resident #1 was a male admitted with diagnoses including a wedge compression fracture, heart failure, stroke, and high blood pressure. His MDS indicated he understood at times, was sometimes understood by others, had a BIMS score of 09, required assistance with ADLs, and was always incontinent of bladder. His care plan and physician orders identified a non-pressure wound to the lower back requiring daily cleansing, collagen, and dressing changes, along with EBP every shift. During the observation, Resident #1 was in bed with a cart in the room containing gloves, masks, and gowns, and a blue name tag was on the door. Treatment Nurse A entered the room, washed her hands, applied gloves, and began wound care. ADON C assisted with turning the resident on his side. Treatment Nurse A removed the old dressing, performed hand hygiene, applied new gloves, cleaned the wound, performed hand hygiene again, applied new gloves, and completed the collagen and dressing application. Neither Treatment Nurse A nor ADON C wore a gown while performing the wound care. In interviews, Treatment Nurse A stated she was supposed to wear a gown and gloves for EBP and said she forgot. ADON C said she had been employed about a week and was not aware she was supposed to wear a gown until after the wound care was completed. The Infection Preventionist stated staff were supposed to wear PPE for residents requiring EBP and that failure to do so could cause infection. The DON and Administrator both stated staff were expected to wear proper PPE when caring for residents on EBP, and the Administrator said not wearing the proper PPE could place residents at risk for cross-contamination or infection.
