Summary

The facility failed to follow proper food handling practices in the kitchen by not consistently labeling food items with the item name and use-by date, not keeping the refrigerator temperature log updated every shift, and not discarding expired food. During observation and interview, meat in the refrigerator was found labeled only with the received date and a use-by date of 4/30/2026, and the Dietary Supervisor stated the ground beef had been pulled out on 5/3/2026 and was not labeled with the food name and the correct use-by date while it was thawing in the refrigerator. The Dietary Supervisor also reviewed the refrigerator temperature log and confirmed that the morning temperature entries for 5/3/2026 and 5/4/2026 were missing. Expired food was also observed in the kitchen. During observation with the Dietary Supervisor, mashed potatoes were found on the kitchen counter with a use-by date of 4/31/2026, and the Dietary Supervisor told the Dietary Aide that the item needed to be discarded. The Dietary Supervisor stated that food items should be labeled with the product name and use-by date, and that thawed food should be labeled and used by the third day. The Dietary Supervisor also stated that food beyond the use-by date needed to be discarded because of the growth of germs and bacteria. The kitchen floor was observed to be wet with puddled water, including water overflowing from the dishwashing machine strainer and leaking onto the floor. The Dietary Aide stated the floor gets wet when dishes are washed in the sink, and the Dietary Supervisor confirmed the floor was wet because water was coming out from the washing machine strainer. The Maintenance Supervisor later observed that the ice machine drainage pipe went directly into the floor and did not have an air gap, stating the pipe was supposed to have an air gap and that it was needed to prevent backflow of bacteria into the ice machine. The Infection Prevention Nurse stated that food and utensils could become contaminated from splatters of water on the floor when water overflowed onto the kitchen floor, and that stagnant water could support bacterial growth.
