Summary

The facility failed to prevent physical abuse for one resident when another resident struck him on the face during an altercation in their shared room. The injured resident had diagnoses of paranoid schizophrenia and anxiety disorder and was documented as cognitively intact on the MDS, with needs for supervision or touching assistance for several activities of daily living. The resident reported that he had been walking in the room, stopped, and looked at his roommate, which made the roommate uncomfortable and led to the roommate hitting him on the right side of the face. The resident who struck the other resident also had diagnoses of paranoid schizophrenia and violent behavior and was documented as cognitively intact on the MDS. His care plan noted a prior physical altercation with another resident in a prior board and care facility because he did not like the other resident, and it included an intervention to help him develop more appropriate coping and interaction methods and to encourage him to express feelings appropriately. During interview, he stated he punched his roommate in the face because he did not like the way the roommate looked at him. The DON confirmed the altercation occurred and stated the injured resident had told staff he had been walking and staring at the other resident, which he believed was why he was hit. The DON also stated the injured resident did not have a care plan for the behavior of stopping and staring at others, and that this behavior should have been care planned because it was triggering for the other resident. The facility’s behavioral assessment and care planning policies stated behavioral symptoms should be assessed, individualized interventions developed, and safety strategies implemented immediately to protect residents from harm, and that care plans should include behavioral descriptions, interventions, rationale, goals, and monitoring.
