Summary

The facility failed to provide suprapubic catheter care to one resident. During an observation in the resident’s room, the suprapubic catheter tubing was noted to have thick discolored white and yellowish growth and sediment extending from the insertion point down to the collection bag, and the bag itself had discolored white to yellow-brown growth and sediment adhered to the sides. During the same observation, an LVN confirmed the presence of white to yellowish growth and sediment in the tubing and bag. A TX stated the catheter appeared not to have been flushed in a long time and needed to be flushed, adding that it looked like it had not been flushed in over a week. The TX later stated she had provided treatment earlier that day but did not look at the catheter tubing or bag and did not flush the catheter. The DON reviewed a photo of the catheter condition and stated staff should look at the entire suprapubic catheter, not just the insertion site, and confirmed the catheter should have been flushed. The resident’s order summary directed flushing the suprapubic catheter with sterile water as needed for obstruction or malfunction, and the MAR directed staff to monitor for signs and symptoms of infection, increased sediment, foul odor, cloudy appearance, catheter-related pain, and urinary retention every shift.
