Delayed implementation of RD tube-feeding recommendations and failure to change enteral feeding supplies on time

Summary

The facility failed to implement the RD’s recommendations for a resident admitted with sacral pressure ulcer, dementia, and protein-calorie malnutrition. The resident’s record showed low body weight and low BMI, and the RD documented on 2/23/2026 that Jevity 1.5 should be increased from 50 cc/hr to 55 cc/hr and Prostat 30 cc daily should be started to address weight loss and support wound healing. The physician orders reflecting these changes were not entered until 2/27/2026. During interview, the RD stated the recommendations were supposed to be carried out within 72 hours, and the DON stated they should be carried out within 3 days, preferably the same day. The RD and LVN both confirmed that the resident’s nutrition-related recommendations from 2/23/2026 were not implemented until 2/27/2026. The resident’s MDS dated 3/10/2026 indicated the resident was cognitively intact, dependent on staff for ADLs, and had one unstageable pressure ulcer. The record review and interviews showed the delay in carrying out the RD’s recommendations for increased tube feeding and Prostat supplementation. The facility also failed to ensure proper management of enteral feeding supplies for another resident with severe cognitive impairment who was dependent on staff for multiple ADLs. During observation, the resident was connected to enteral feeding that was labeled with a date of 4/26/2026 and time of 9 p.m., and the feeding was turned off. An LVN stated that G-tube feeding should have been changed and labeled with date and time because staff would not know when to change it, and that using G-tube feeding longer than 24 hours can cause abdominal discomfort or infection. The DON stated that enteral formula must be discarded after 24 hours of opening and the tubing must be changed after 24 hours of opening.
