Summary

The facility failed to ensure the Lead Respiratory Therapist maintained accurate and complete competency records for three sampled staff members, including a Registered Nurse and two Respiratory Therapists, for ventilator and handheld nebulizer (HHN) care. During record review, the Ventilator and HHN competency checklists for each of the three staff were signed and dated, but the sections documenting whether each staff member had demonstrated proper knowledge, use, and application of the ventilator and whether they could safely and effectively perform HHN therapy were left blank. During interview, the Lead RT stated he left those competency questions blank even though he believed the staff were competent. The facility also failed to ensure the Lead RT documented the exact date of observation for tracheostomy tube change competency. The Subacute Skills Checklists for the same three staff indicated that the Lead RT observed multiple tracheostomy tube changes, including simulator use and resident tube changes, but each checklist listed only one date for all observations. Review of the tracheostomy tube change schedules showed that only one resident was scheduled for a tube change on each of the dates referenced, not three resident observations. During interview, the Lead RT stated he documented one date for all four observations and acknowledged that the dates of observation were inaccurate. The DON stated the competency checklists should have been completely answered and the exact dates of tracheostomy tube change observations should have been documented. The DON also stated that incomplete competency documentation would show that the staff were not trained and competent on ventilator, HHN, and tracheostomy tube changes and could create confusion in staff records. The Lead RT's job description identified responsibilities for training employees and demonstrated accuracy and thoroughness.
