Infection Control Failures With Isolation Laundry, Laundry Room Separation, and Scabies Reporting

Summary

The facility failed to maintain an effective infection prevention and control program when staff did not consistently separate isolation and non-isolation laundry and linens. During observation in a soiled utility room, a CNA placed Resident 2’s soiled linens into a red bin that staff identified as being for non-isolation residents, even though Resident 2 was on contact isolation for shingles. Staff also observed that the soiled utility room did not have a designated isolation barrel available at that time, and multiple staff stated that isolation linens were supposed to be kept separate from non-isolation linens. The resident’s order summary reflected strict single-room isolation with contact precautions related to shingles. Laundry handling in the facility also did not maintain separation between clean and dirty items. In the laundry room, a blue barrel containing overflowing clean linens was left in the dirty area beside the red bin used for dirty soiled linens. The laundry supervisor confirmed the laundry room had separate clean and dirty sides and stated clean linens should not be left in the dirty area. The laundry staff member observed stated the blue barrel should not have been left on the dirty side of the laundry room. The facility also did not report suspected scabies cases to the local health department and CDPH when residents were clinically diagnosed by a dermatologist with suspected scabies. Residents 3, 4, 5, 6, and 7 had rashes and itching documented in SBAR forms and were later clinically diagnosed with suspected scabies by the dermatologist, while Resident 8 was assessed by a wound doctor with suspicious scabies. The infection preventionist stated the facility waited for a confirmed case before reporting and did not report the suspected cases when they were clinically diagnosed. The DON stated suspected scabies should have been reported. In addition, contact isolation was not initiated when rash symptoms first developed for Residents 3, 4, 5, 6, and 7; instead, isolation was started only after the dermatologist’s suspected scabies diagnosis was made.
