Expired Insulin Kept in Medication Cart

Holiday Manor Care CenterCanoga Park, California Survey Completed on 05-21-2026

Summary

The facility failed to ensure an expired Lantus Insulin pen for Resident 30 was removed from Med Cart 2 during a medication cart inspection. During the concurrent observation and interview, an LVN found the insulin pen stored in the cart with an open date, and stated that Lantus insulin should only be used for 28 days after opening. Resident 30 was admitted with diagnoses of essential hypertension and type 2 diabetes mellitus. The resident’s MDS indicated the resident could make self-understood and understand others, and required partial/moderate assistance with oral hygiene, toileting hygiene, showering, lower body dressing, footwear, and personal hygiene. During interview, an RN stated that expired medication should not be kept in the medication cart because it would no longer be effective, and the facility policy stated that opened multi-dose vials are dated and discarded within 28 days unless the manufacturer specifies otherwise.
