Kitchen Equipment Not Maintained in Clean, Safe Condition

Summary

The kitchen ice machine had black particles on the inside of the machine when observed with the Dietary Supervisor, and the napkin used to wipe the interior sides and metal portion in the middle of the machine showed the particles. The Dietary Supervisor stated the particles should not be present and that the machine should be clean so bacteria would not be present, but she was not sure why the particles were there. The Dietary Supervisor and Maintenance Director stated the ice machine was used that morning to place ice in bins to keep milk and juice cold for residents, and the Maintenance Director stated the machine had last been cleaned approximately two weeks earlier, with the monthly cleaning log showing cleaning on 4/29/2026. The ice machine scoop had dark spots, and the Maintenance Director stated the spots were from tarnish and that the coating had come off because staff hit the metal sides and middle metal part with the scoop. Three cutting boards, two green and one brown, were observed with cracks and scratches, and the Dietary Supervisor stated they should be discarded and replaced. A pair of oven mitts also had dark spots, and staff stated that when the mitts are too dirty they are discarded; the Dietary Supervisor stated she was going to replace them. The facility policy required the ice machine to be cleaned and sanitized monthly, internal components to be cleaned monthly or per manufacturer recommendations, and kitchen equipment to be maintained in good working order.
