Summary

The facility failed to develop and implement a comprehensive person-centered care plan for Resident 12 related to visual impairment. The resident was admitted with diagnoses including essential hypertension and blindness in both eyes, and the MDS dated 4/18/2026 indicated severely impaired cognitive skills for daily decision making and total dependence on staff for multiple activities of daily living. The MDS Section V identified visual impairment as a triggered care area, but during interview RN 2 stated that no care plan had been developed because it was missed by the MDS nurse. The facility also failed to develop and implement a care plan for Resident 1 related to the risks associated with Seroquel. Resident 1 had diagnoses including unspecified psychosis not due to a substance or known physiological condition and Alzheimer’s disease, and the MDS indicated severely impaired cognitive skills for daily decision making with supervision or touching assistance needed for several activities of daily living. During interview and record review, RN 2 stated there was no care plan addressing the black box warning for Seroquel, and that medications with a Black Box Warning should have specific monitoring of potential adverse effects, including suicidal ideation, to ensure proper interventions are in place. The facility further failed to develop and implement a care plan addressing Resident 79’s hyperglycemia. Resident 79 had diagnoses including type II diabetes mellitus with diabetic neuropathy and unspecified atrial fibrillation, and the MDS indicated severely impaired cognition. The order summary included Accuchecks twice daily for diabetes and to call the MD if blood glucose was greater than 200 mg/dL. The eMAR showed a blood glucose level of 302 mg/dL on 5/9/2026, and the DON stated that Resident 79 did not have a care plan addressing hyperglycemia with appropriate interventions.
