Failure to Supervise High Elopement-Risk Resident

Summary

The facility failed to provide appropriate supervision and elopement prevention for a resident who was identified as a high elopement risk. The resident had diagnoses including sepsis and dementia, and the Elopement Risk Assessment dated March 3, 2026, identified dementia, wandering behaviors, and high elopement risk. The MDS dated March 12, 2026, also indicated cognitive loss or dementia. Nursing notes from April 4 through April 16, 2026, described the resident as frequently walking throughout the facility, confused, difficult to redirect, and showing escalating behavioral concerns. On April 16, 2026, staff last observed the resident in different areas of the facility during the morning, but no staff saw the resident leave the building. The Admin Secretary stated the resident was seen in the lobby before she stepped away from the front desk and was gone when she returned. The assigned CNA stated he last saw the resident in her room and did not see her afterward. The LVN stated the resident wandered frequently and that staff were expected to monitor residents and ensure safety. The DON stated staff relied primarily on frequent checking and redirection, and that no staff were in the lobby or outside the front entrance when the resident left. During observation, the facility’s front entrance and back door were unlocked and had no alarms, and no staff were present at either location. The DON confirmed the elopement IPOC, dated April 17, 2026, only included a general intervention to structure the day to monitor activities and safety, with no other specific interventions and no clear guidance for staff to prevent elopement. The DOR later observed the resident outside the facility, unsupervised, walking down a hill on the street, appearing confused, and then falling backward. Emergency services were contacted, and the resident was transferred to the hospital, where the admission H&P documented that the resident had fallen after eloping from the CCC and sustained a left leg fracture.
