Summary

Resident 1 received more insulin Lispro than ordered by the physician. The physician’s order dated 4/14/26 directed that before meals, if the blood sugar was 151-200, the resident was to receive 2 units of HumaLog insulin, and if the blood sugar was 70-150, no insulin was to be given. The medication administration record for April 2026 documented a blood glucose result of 155 on 4/23/26, and the resident was administered insulin at 1130 hours. The facility’s eINTERACT Change in Condition Report for 4/23/26 documented that Resident 1 received more insulin than ordered, and the nursing addendum stated the resident received 4 units of insulin Lispro instead of 2 units per the sliding scale order. During interview and concurrent record review on 5/14/26, RN 2 stated a medication error occurred when Resident 1 received 4 units instead of the ordered 2 units for a blood sugar of 155. RN 2 stated the physician was notified immediately and that the resident was ordered to have blood glucose monitored every 30 minutes for four hours after the incident. RN 2 also stated the documentation system defaulted to the sliding scale order, which prevented documentation of the 4 units administered. The facility’s insulin administration policy stated the insulin type, dosage requirements, strength, and method of administration are to be verified with the order on the medication sheet and the physician’s order before administration.
