Summary

The facility failed to implement appropriate infection prevention and control practices when soiled linens from a resident on Enhanced Barrier Precautions were handled in a shared bathroom used by another resident. During observation, a CNA provided care after a bladder or bowel accident, changed the resident’s soiled bed linens, entered the room bathroom, and placed the soiled linens directly on the bathroom sink. The CNA stated that placing soiled linens on the shared sink created a risk of cross-contamination and acknowledged that the other resident used the same bathroom and had confusion and forgetfulness that could affect understanding and following infection control practices. The resident on Enhanced Barrier Precautions had diagnoses including infection of the amputation stump, vancomycin resistance, and bilateral below-knee amputations, and the provider had ordered EBP during high-contact care activities due to an open wound. The roommate independently used the shared bathroom and had diagnoses including cerebral infarction, type 2 diabetes mellitus, vascular dementia, chronic viral hepatitis C, visual field defects, and protein calorie malnutrition. A nurse stated that soiled linens should not be placed on any surfaces and should be placed in a protective bag if they needed to be set down before going into the dirty linen hamper. The ADON stated that the resident was on EBP because of increased risk for infection transmission and that placing the soiled linens on the bathroom sink created an infection control risk.
