Incorrect Isolation Signage and PPE Breach During Contact Precautions

Summary

Standard infection prevention and control practices were not followed for two residents on contact isolation. Resident 12 had an admission record showing respiratory failure, severe impairment in daily decision-making on the MDS, and dependence for eating, oral hygiene, toileting hygiene, showering, upper and lower body dressing, and personal hygiene. The resident’s IDT documentation dated 4/15/2026 indicated contact isolation, and the care plan for actual infection dated 4/17/2026 included instructions to monitor infection control measures and contact precautions. During observation on 4/25/2026, the signage outside Resident 12’s room indicated enhanced barrier precaution instead of contact isolation. RN 5 verified that the posted sign was EBP signage. During record review and interview with the IPN, it was confirmed that Resident 12 was on contact isolation for CRE and VRE, and that the sign outside the room should have been a contact isolation sign to alert staff and visitors to wear appropriate PPE before entering the room. Resident 36 also had respiratory failure, severe impairment in daily decision-making, and dependence for eating, oral hygiene, toileting hygiene, showering, upper and lower body dressing, and personal hygiene. The care plan for actual infection included contact precautions, hand washing before and after patient care, and PPE per facility protocol. During a medication administration observation on 4/27/2026, LVN 3 exited Resident 36’s room with PPE on and walked across the hallway to return the vital signs machine, stating that PPE was not changed because the resident’s heart rate was checked quickly and the machine had to be taken out before medications were passed. RN 3 and the IPN both stated that staff should doff PPE before leaving the room and don new PPE before re-entering, and the facility’s P&P stated that attire and equipment used for patients in isolation are not to be worn outside the isolation room.
