Summary

The facility failed to ensure care plans were revised and interventions were followed for two residents. The report states that the comprehensive care plan was not kept current when resident conditions and refusals changed, and that care plan interventions were not consistently carried out as documented. For one resident, the care plan identified risk for constipation related to decreased mobility, diminished appetite, medication side effects, poor fiber intake, and poor fluid intake, and included interventions such as monitoring for constipation, increasing fiber and fluids, encouraging activity, and offering bowel medications as needed. The resident was also receiving hydrocodone-acetaminophen for pain, with bowel monitoring included in the care plan. Nursing documentation showed multiple days with no bowel movements recorded, and nursing notes documented that the resident refused bowel regimen medications and stated she had a loose stool after dialysis. The DON confirmed the resident had multiple refusals of the bowel regimen and that the care plan was not revised to reflect those refusals. For the second resident, the care plan for pain required documentation of nonpharmacological interventions before giving PRN hydromorphone. The physician order listed alternatives such as repositioning, distraction, breathing techniques, gentle massage, relaxation, music, and other measures. The MAR showed hydromorphone was administered multiple times, but the nonpharmacological interventions were documented as NA. During interview, the resident stated the pain medication made him constipated until he finally chose to use an enema. The ADON and DON reviewed the record and confirmed that NA was documented and that there was no documentation showing which nonpharmacological interventions had been attempted before the PRN medication was given.
