Pressure Injury Assessment, Treatment, and Wound Care Infection Control Failures

Summary

Pressure injury care was not implemented according to facility policy and national guidance for two residents. One resident was admitted with an unstageable sacrococcyx pressure injury and had a care plan directing weekly skin evaluation and documentation of wound measurements, tissue type, exudate, and other changes. The resident’s skin checks showed the sacrococcyx wound progressing from a 5 cm by 5 cm wound with 100% slough to a 6 cm by 6 cm by 3 cm wound with undermining and exposed tissue, but the March 17 assessment did not describe the remaining 40% of the wound bed. The treatment nurse stated he did not stage the pressure injury and waited for the wound care specialist, and the DON stated licensed nurses should be able to stage pressure injuries and that she had to research the criteria used for staging. The wound care nurse practitioner stated the sacrococcyx wound should have been staged as a stage 4 pressure injury when muscle and bone were exposed, rather than remaining unstageable. The same resident also had stage 2 pressure injuries on both buttocks, and the facility’s quick reference indicated stage 2 pressure injuries were to be treated with Dermafilm. The DON and wound NP stated the barrier cream used on the bilateral buttocks wounds was not appropriate for an incontinent resident and that the wounds should have been covered with a foam dressing instead of left open to air. For another resident with venous ulcers to both lower legs, wound care was observed and infection control practices were not followed. The treatment nurse removed dressings and placed the left leg directly on a pillow without a clean field, used the same scissors to cut the new xeroform dressing that had been used to remove the dirty bandage without disinfecting them, and used the same scissors again for the right leg. The nurse stated he should have provided a clean area for dressing removal and redressing and should have disinfected the scissors before using them on a clean dressing. The resident’s physician order directed cleansing the bilateral shin ulcers with normal saline, applying xeroform to open areas, and covering with non-stick gauze and kerlix.
