Summary

The facility failed to ensure Resident 21’s medical record documented that the formulation of an advance directive was discussed. Resident 21 was admitted with diagnoses including COPD, dementia, and anxiety disorder. The admission H&P stated the resident did not have the capacity to understand and make decisions. The MDS dated 2/1/2026 indicated severely impaired cognition, that the resident was sometimes able to understand others and make needs known, and that the resident’s advance directive was not completed. The ADON stated the form should have been completed to show that the AD was discussed and offered, written information was provided, or the reason it was not completed.
