Summary

The facility did not ensure a resident with a left arm sling received timely skin monitoring and pressure injury prevention measures, resulting in an unstageable pressure injury to the left elbow that later declined to a stage 4 pressure injury with exposed hardware. The resident was admitted with a left humerus fracture, severe cognitive impairment, Parkinson’s disease, dementia, severe protein-calorie malnutrition, cachexia, weakness, and a history of a coccyx deep tissue injury that later healed. The resident was dependent on staff for repositioning, dressing, hygiene, transfers, and toileting, and was assessed as high risk for pressure injury development with a Braden score of 10 and a weight of 77 pounds. The resident’s care plan addressed pressure injury risk in general, including skin observation, pressure-relieving devices, low air loss mattress use, Prevlon boots, and turning/repositioning, but it did not include interventions for monitoring the skin under the left arm sling or guidance about sling removal or protection of the left arm. Physician orders included weekly skin checks and a sling to the arm every shift for fracture, but there was no order directing daily or every-shift skin monitoring under the sling or specifying whether the sling could be removed. Nursing documentation noted the sling was in place, but did not document that the skin under the sling was checked or that the skin remained intact. The left elbow pressure injury was first identified during a wound assessment when the resident was rolled for coccyx care and a black area was noted at the left elbow. The wound MD documented an unstageable pressure injury with eschar and ordered treatment and an x-ray to rule out osteomyelitis, which was negative. The wound later worsened and was documented as a stage 4 pressure injury with protrusion of prosthetic/hardware and necrotic tissue. Staff interviews confirmed that monitoring under the sling had not been added to the care plan or documented until after the wound had already been identified and had declined, and the resident’s sling use had not been accompanied by documented skin checks under the sling before the injury was found.
