Summary

The facility did not establish and maintain an infection prevention and control program based on current standards of practice. The report states that the facility was not monitoring infections, tracking and trending infections, or completing monthly infection rates for 2025. In November 2025, there was no documentation of infection monitoring, surveillance, tracking and trending, or infection rates. The facility also had a COVID-19 outbreak in September 2025 that was not thoroughly investigated, and the infection preventionist stated there were no infection surveillance logs prior to January 2026. The infection preventionist, who began in January 2026, stated she had asked the NHA for surveillance logs and infection prevention documentation and was told none existed before her start date. She stated infection tracking was not being performed before she began tracking infections daily in January 2026. The NHA stated the previous DON had been completing infection preventionist tasks, but acknowledged that the facility was not monitoring infections, tracking and trending infections, and had not completed monthly infection rates prior to January 2026. The 2025 infection prevention binder reviewed by surveyors confirmed the absence of the required surveillance and trending documentation. The report also documented failures related to transmission-based precautions and PPE use. A laundry room did not have a sink, and laundry staff used a sink across the hallway, with the laundry room door closed and staff needing to touch the door handle before hand hygiene. For one resident with a G-tube, a nurse administered medications through the tube while wearing gloves but not a gown, despite an EBP sign posted outside the room. For another resident on enteric precautions, staff entered the room and provided care without gowns or gloves on multiple occasions, including helping the resident sit up in bed and dressing the resident, even though signage outside the room instructed staff to wear PPE.
