Summary

The facility did not ensure that a resident with a diagnosis of PTSD received trauma-informed, culturally competent care that accounted for his experiences and preferences and identified triggers to reduce re-traumatization. The resident was admitted with PTSD, had a BIMS score of 15 indicating intact cognition, and his MDS identified PTSD. However, his comprehensive care plan only referenced PTSD in a malnutrition-related focus and did not include a PTSD-specific focus, goal, or interventions, and there was no documentation of his triggers in the care plan. Record review showed the resident had discussed his PTSD with psychiatry and other providers. A psychiatry note documented depression, anxiety, sleep disturbance, and trauma related to his work as a police officer, including that his partner was shot and died in his arms and that he himself had been shot on duty. The resident declined psychiatric medication, sleep medication, and therapy at that time. Additional notes documented that he reported PTSD-related issues at church, became depressed after a two-day pass from the facility, and was restless, crying, talking, and hollering in his sleep. During interview, the resident stated that hearing other residents yell for help in the hallway triggered his PTSD and that he was becoming more depressed after his wife’s recent death. Staff interviews showed mixed awareness of the diagnosis but no knowledge of his specific triggers. Several CNAs and LPNs stated that PTSD information and triggers should be in the care plan or chart, while the DON stated the resident’s PTSD and specific interventions and triggers should have been added to the care plan so staff would know what to monitor. The facility therefore failed to include the resident’s PTSD and trigger-specific interventions in the comprehensive care plan despite the diagnosis being known and documented.
