Failure to Monitor Resident After Cocaine Use on Leave

Summary

The facility did not ensure adequate supervision and safety measures for a resident with a history of substance use disorder after the resident returned from leave and admitted to using cocaine while away from the building. The resident, who was cognitively intact with a BIMS score of 15, had diagnoses including alcohol abuse and a history of PTSD, depression, and recent grief related to his wife’s death and other personal stressors. After the resident returned, staff documented that he had been depressed, had not eaten for several days, and had reported doing "not so good things" while out of the facility. The record showed that a urine drug screen was obtained and was positive for cocaine. The resident’s progress notes also reflected that he had been out of the facility for periods of time, had not always signed out, and had left without medications on at least one occasion. A nurse practitioner noted increased depressive symptoms, abnormal mouth movements, possible cognitive impairment or altered mental status, and ordered the urine drug screen to evaluate possible substance involvement. The resident later stated that he had used drugs one time and denied ongoing illicit drug use. Although the facility policy for Substance Use Disorder stated that residents’ substance use history and overdose risk should be identified and documented, and that care plan interventions may include monitoring for signs and symptoms of substance use and overdose, especially after return from leave, the staff interviews showed no ongoing monitoring was in place for this resident at the time of the survey. Multiple CNAs stated they were not aware of any monitoring being done, and an LPN and the ADON acknowledged that specific monitoring and care plan interventions should have been included. The DON stated the substance use issue had only recently been added to the care plan and agreed it should have been addressed earlier, including monitoring for signs of overdose.
