Infection Control Program and Procedure Failures

Summary

The facility did not establish and maintain an infection prevention and control program designed to provide a safe, sanitary, and comfortable environment and help prevent the development and transmission of communicable diseases and infections. The report states the facility also did not conduct an annual review of its IPCP and update the program as necessary. The facility’s Water Management Program team, documented as of August 2025, included the NHA and Director of Maintenance, but did not include the IP. During interview, the ADON/IP stated she was not part of the Water Management Program team, and the DON stated he would expect the IP to be part of that team. The facility’s infection control policies were not all reviewed annually. The Legionella Surveillance Policy and Procedure was dated 10/1/19, and the Hand Hygiene/Handwashing Policy was dated 2023. The ADON/IP stated that infection control policies should be reviewed annually and acknowledged these policies should have been reviewed. The DON also stated that all infection control policies should be reviewed annually and said there was no documentation of the Hand Hygiene/Handwashing and/or Legionella Surveillance policies being reviewed in the QA program. During medication administration, an LPN failed to clean the rubber cap of an insulin pen with alcohol before attaching the needle when administering NovoLog FlexPen insulin to a resident with type 2 diabetes mellitus with diabetic neuropathy. The LPN completed hand hygiene, checked the expiration date, removed the cap, and attached the needle without cleaning the pen’s rubber stopper. The LPN stated she had never been taught to do that, and the DON agreed the insulin pen should have been cleaned with an alcohol swab before the needle was attached. During wound care for another resident, the ADON prepared the wound care area by placing a pillow under the resident’s foot, removed wet gauze from a cup, dropped it onto the pillowcase taken from the resident’s bed, picked it up from the pillowcase, and used it to clean the wound. The ADON later stated the gauze had been dropped on the pillow from the resident’s bed and acknowledged it could be considered cross contamination; the DON stated staff should perform wound care in a clean field.
