Summary

The facility did not ensure necessary care and services were provided to prevent pressure injuries from developing and to support healing for one resident with paraplegia, morbid obesity, and dependence on staff for bed mobility. The resident was admitted with intact cognition, was totally dependent with physical assist of 2 for bed mobility, and had a care plan identifying risk for altered skin integrity related to paraplegia, obesity, and the need for staff assistance with positioning. The care plan called for hourly side-to-side repositioning and a pressure-reducing mattress set at 350, and the resident had an order for staff to check the setting and function of the air mattress each shift. The resident developed an unstageable deep tissue injury on the sacrum and bilateral buttocks, with a skin assessment documenting a deep tissue injury on the sacrum and a later provider note stating the wound physician debrided the sacral and bilateral buttock wound at the bedside. The resident’s MDS assessment indicated the resident was not on a turning/repositioning program. Staff interviews showed nursing staff did not document repositioning, despite acknowledging the resident needed assistance to reposition because of paraplegia, obesity, a cervical neck collar, and the need for the head of bed to remain at least 30 degrees. The resident’s TAR also lacked documentation that the air mattress setting and function were checked on multiple shifts between 4/10/26 and 4/19/26, even though staff stated this was expected each shift. The DON acknowledged the missing documentation and stated the facility did not have a documentation policy, while the ADON stated staff do not document repositioning because it is considered standard of care. The report also notes the resident had a skin tear on the right buttock and later wound care orders for the bilateral buttocks and posterior inner left thigh.
