The Director of Labor Management acknowledged the low weekend staffing alert and stated the facility struggled to enter agency staff, especially on weekends, because the office was closed and documentation was not always sufficient to reconcile punches and capture agency hours. The facility was not able to provide additional information explaining the inaccurate staffing submission by the end of the survey.
