Unsafe Food Handling and Improper Pureed Diet Consistency

Summary

Food was not prepared and served in accordance with professional standards during lunch service on the third floor dining room. The steam table with meal items already in metal bins was brought from the kitchen to the kitchenette area, but the food was not observed to be temped before service and the steam table was not plugged in or turned on. Cook-L served soup, grilled cheese sandwiches, taco salad, mixed vegetables, and pureed items to residents while using gloved hands to touch resident meal tickets, cabinet doors, the outside of the metal bins, the top of the steam table, and the microwave handle and buttons. No hand hygiene was observed, and when gloves were changed, hand hygiene was not performed before new gloves were put on. During the meal service, residents complained that all of the food on the plate was cold, and Cook-L placed the plate in the microwave to heat the meal. Cook-L stated the food is temped in the kitchen when placed in the steam table, but it is not re-temped on the floors, and also stated the steam table was a brand-new unit that had never been plugged in. Cook-L initially plated grilled cheese sandwiches by hand before later using tongs, and also touched the mixed vegetables and taco salad shell with gloved hands while arranging the plates. The meal service included pureed taco meat and pureed mixed vegetables for residents on a pureed diet. The pureed foods were not prepared to the required consistency. The pureed taco meat was observed to be the consistency of runny gravy, and the pureed mixed vegetables were thick and chunky rather than smooth. The facility policy and MealSuite Diet Manual state that IDDSI Level 4 pureed foods should be smooth, cohesive, homogenous, and free of lumps. The Kitchen Manager stated the lunch cook purees the meal before lunch service, that the recipe should be followed every time, and that the pureed food should be smooth with no lumps; the Kitchen Manager also stated the lunch meal should have been a pudding-like consistency and acknowledged the concern about the food not being at the right consistency.
