Summary

The facility failed to provide appropriate pressure injury care for a resident who was admitted with CHF, stage 3 CKD, protein-calorie malnutrition, and a Braden score of 17 indicating risk for pressure injuries. On admission, the resident’s skin assessment documented redness and a Stage 1 area on the coccyx, but the baseline care plan included only barrier cream with no instructions for use and no other pressure-relief interventions. The wound MD was not notified of the admission coccyx finding, and the resident did not have a treatment order initiated for the Stage 1 area. The facility also did not complete a thorough wound assessment with staging, wound-bed description, or measurements beyond noting redness. The resident’s condition worsened within less than two weeks of admission. When staff later identified a coccyx wound, the documentation described a 1 x 1.5 wound, but the wound was not thoroughly assessed at that time and was not staged or described with wound-bed characteristics. The wound MD then evaluated the resident and documented an unstageable coccyx pressure injury that was 100% necrotic. Although the facility initiated skin integrity and pressure injury care plans after the wound worsened, those plans did not include all of the interventions identified by the wound MD, including offloading measures, a pressure-relieving device in bed or chair, limiting time in the chair, or a cushion for the recliner. Survey observations showed the resident sitting in a recliner for hours without a pressure-relieving cushion and later lying in bed with heels resting directly on the mattress. The resident reported the mattress was hard, that movement caused shortness of breath, and that there was no cushion in the recliner. Facility staff acknowledged the resident should have had interventions such as wound nurse and wound MD notification, care planning, and treatment orders when the Stage 1 area was found, but those actions were not completed at the time. The wound MD also recommended an air mattress, but the resident continued on the regular mattress until the air mattress was placed several days later.
