Summary

The facility failed to ensure that residents with pressure injuries received necessary treatment and services consistent with professional standards of practice to promote healing and prevent new pressure injuries from developing. Surveyors identified deficiencies involving four residents with pressure injuries, including one resident cited at scope and severity G and three residents cited at scope and severity D. The facility’s own policy stated that individualized interventions should address risk factors, be implemented as ordered, and be monitored for effectiveness with modifications when a new pressure injury develops or when wounds do not progress toward healing. One resident was readmitted with an unstageable deep tissue injury to the left heel and had heel boots ordered as part of the care plan. Surveyors observed on three separate occasions that the resident’s heels were not offloaded and the heel boots were not in place while the resident was in bed; the boots were found in the wheelchair instead. The wound RN stated the heel injury had increased in size after discovery, and the DON confirmed the resident was supposed to have heel boots on at all times except during care. The wound RN also stated that if heel boots were unavailable, heels could be offloaded with pillows until the boots were available. Another resident had a stage 4 sacral pressure injury that worsened and became infected, resulting in osteomyelitis and hospitalization for IV antibiotics. The facility NP documented that the resident had chronic loose stools and needed frequent bed changes to prevent wound breakdown, but that recommendation was not added to the care plan. A third resident experienced significant weight loss over several months, but the facility did not reassess whether the resident’s pressure injury prevention interventions remained effective after the weight loss. That resident later developed a left buttock pressure injury that progressed from a small open area to stage 4, with later imaging confirming osteomyelitis. A fourth resident was admitted with a pressure injury that became infected and required hospitalization and antibiotic treatment.
