Summary

The facility failed to assess and care plan a positioning device, a foam wedge used to keep Resident 16's head raised, for 1 of 3 sampled residents reviewed for positioning and mobility. Resident 16 had diagnoses that included dementia and difficult swallowing, was severely cognitively impaired, and required substantial assistance for rolling to either side and returning to the back when in bed. The 04/27/2023 care plan documented that the resident needed assistance with ADLs, had esophageal reflux, was at risk for pressure ulcers and falls, and staff were to wait at least one hour after meals, then lay the resident down between meals, keep the head of bed elevated, and assist with turning and repositioning. No other positioning devices were documented in the plan of care for use in bed. On 04/27/2026, Resident 16 was observed sleeping in bed with the bed flat and a foam wedge positioned behind the resident's back; the wedge was as wide as the bed and extended from the resident's head to mid-back. The resident was observed on 04/28/2026 in the same position, leaning toward the left edge of the wedge, and on 04/29/2026 the resident's head had rolled off the wedge to the left with the neck bent forward in an awkward position. On 04/30/2026, the resident was again in bed on the wedge and was cued by an unidentified NAC to scoot back over. Staff interviews indicated the wedge had not been implemented by therapy, had been in place for months, and had not been care-planned; staff also stated it was used to keep the resident's head elevated because of swallowing-related risks. The regional clinical services director stated they were not aware the wedge was being used and was unsure why it was necessary, noting the same effect could be achieved by raising the head of the bed.
