Summary

The facility failed to store, prepare, distribute, and serve food in accordance with professional standards for food service safety in the kitchen. During an observation of the kitchen, 41 of 58 plastic coffee cups on the clean rack were stained, had sediment in them, and had scratches on the bottoms and sides; one cup also had lipstick on the rim. A 1-gallon bag of frozen fish in the freezer was not sealed, and a chipped and worn spatula was hanging on the clean rack next to new spatulas. The observation also found personal items in a bin in the refrigerator with resident food, along with 2 packages of meat items that were unlabeled and unsealed, a variety of unlabeled vegetables in a plastic bag, a 32-oz jar of spaghetti sauce, and a 32-oz jar of mayonnaise. One can of soda and a package of store-bought pie crusts were on the shelf near the bin. Three 1-oz containers that appeared to contain olives and jalapenos were not sealed tightly, dated, or labeled. The ice machine was dripping rapidly onto the floor from the outside and inside of the machine, with water dripping from the ice chute, around the door, and onto the floor; a black substance was noted where the drips were falling from onto the ice. In interviews, the DM said the dishwasher was responsible for making sure dishes were clean before they were placed on the clean rack, and the cook and dietary aides were responsible for properly storing food in refrigerators, freezers, and the storeroom. The DM stated the personal items in the refrigerator belonged to the cook and should not have been there because transfer of germs could cause cross-contamination and make residents sick. The CK acknowledged keeping personal food and drink in the resident refrigerator, said she knew about cross-contamination, and stated her items were not labeled and dated. The MS said he had not been informed about the leaking ice machine and needed to take it apart to determine the cause of the heavy condensation.
