Summary

The facility failed to ensure a successful elopement was reported to the State Agency within 2 hours of its occurrence. The facility policy titled, "Abuse, Neglect, Exploitation & Misappropriation," stated that neglect includes failure to adequately supervise a resident known to wander from the facility without staff knowledge, and that any employee or contracted service provider who witnesses or has knowledge of abuse, neglect, exploitation, mistreatment, injuries of unknown source, or misappropriation of resident property must report it immediately, but no later than 2 hours after the allegation is made. Resident R5, who had diagnoses including cerebral infarction, traumatic subdural hemorrhage, and altered mental status, was admitted with severe cognitive impairment; the MDS showed a BIMS score of 0 out of 15 and continuously present disorganized thinking. On 05/10/26, R5 had a successful, unwitnessed elopement from the facility and was found on the sidewalk off the premises next to an active highway. Progress notes indicated the supervisor was notified after the resident was located and returned, and the Administrator later stated he had been made aware of the elopement but initially thought it was an assisted exit and was only reporting it at the time of interview.
