Water Management Program and Eye Drop Administration Infection Control Deficiencies

Summary

The facility failed to implement an effective and comprehensive water management program intended to identify, monitor, control, and reduce hazardous conditions supporting the growth and spread of Legionella within the facility water system. Review of the revised January 2024 Water Management Program policy showed that while the program identified a water management team, recognized the facility’s high-risk resident population, and listed potential sources and conditions that could promote Legionella growth, it did not establish measurable control limits for temperature, disinfectant levels, pH, or other monitoring parameters. The policy also lacked defined corrective actions for abnormal findings or positive Legionella results, and the facility did not provide evidence of routine monitoring, maintenance records, water quality testing, flushing schedules, completed logs, validation or verification activities, staff training, assigned responsibilities, competency, or a detailed hazard analysis and building-specific risk assessment. Interview with the Maintenance Director and Regional Director on May 7, 2026 confirmed that the current program did not meet required standards. Both individuals acknowledged understanding of the requirements moving forward but were unable to describe the building’s water system, identify potential high-risk hot spots, or explain what testing and preventive measures were in place to monitor and control Legionella risk. In a separate observation of Resident R101 during medication administration, Licensed Nurse E4 administered Muro 128 Ophthalmic Solution 2% to the resident’s left eye while wearing gloves, then did not remove the gloves or perform hand hygiene before obtaining the resident’s blood pressure and touching the blood pressure machine with the same gloves. The nurse also placed the resident’s eyeglasses back on while still wearing the same gloves and continued medication administration, including oral medications, without changing gloves. The nurse confirmed that gloves were not changed during the medication administration process.
