Summary

The facility failed to maintain an infection prevention and control program because its laundry process was not functioning as intended and clean, sanitized linens were not consistently available for residents. The report states that the facility’s two washing machines were not working for three to four weeks, while the dryers were functioning. Staff reported that there was not enough linen for resident care, and laundry was being taken once a day to be washed because of the broken machines. One resident was observed lying on a bare mattress without a pillow or bed linens in the room. During observations and interviews, laundry handling practices were inconsistent with the facility’s own policies. Staff stated they were using the two resident laundry machines so staff would have some linen available, and laundry was not covered while being moved between areas or delivered to nursing units. The laundry room had a musty odor, and laundry chemicals were stored on the floor between the washers in their manufacturer containers. Staff also reported mixing laundry detergent and de-stainer in an empty juice container or an unmarked resident water pitcher, estimating amounts by eye or using half a pitcher per load. The EVS Director stated he transported soiled linens in trash bags in his personal vehicle to a laundromat and later to a sister facility, and that some linens were stained and moldy from sitting so long. The report also documents additional infection control concerns in resident care areas. Red bins used for enhanced barrier precautions did not contain red bags, and the Interim DON confirmed hazardous waste was being thrown in the dumpster with regular trash. A staff member reported receiving a clean gown with dried feces smeared on it, and another observation found a clean gown with dried feces that fell onto the floor. Infection tracking and line listing were incomplete, with residents marked as “other” without clear identification of the infection. The facility identified active infections in April and May 2026, including HIV, multiple viral hepatitis cases, and various bacterial infections, while the Immediate Jeopardy was determined to have begun on 4/17/26.
