The facility failed to ensure that necessary treatment and services were provided consistent with professional standards of practice for two residents reviewed for unnecessary medications. Facility policy required regular consultant pharmacist services, including identification and evaluation of medication-related issues and reporting of medication errors. The resident later had a gabapentin order discontinued when hospitalized and then restarted as an active order. During interviews, the Regional Director of Clinical Operations stated she was unsure why the first resident’s pharmacist recommendation was not responded to timely and why the second resident’s recommendation was not transcribed, despite physician agreement.
