Summary

The facility failed to provide respiratory care and oxygen services consistent with professional standards of practice for one resident with obstructive sleep apnea, dementia, and heart failure. The resident had a physician order for CPAP use at bedtime, with the original order dated January 29, 2026 and a current order dated April 28, 2026 for CPAP settings of 18.1 pressure from 9:00 PM to 6:00 AM. Facility policy for CPAP/BiPAP support required the machine to be wiped weekly, the humidifier to be cleaned weekly and air dried, and the mask, nasal pillows, and tubing to be cleaned daily and placed in a plastic bag after drying. Observations of the resident's room on May 17, May 18, and May 19, 2026 showed the CPAP mask lying on the nightstand and appearing slightly greasy on the inside. The resident's physician orders from January 29 through May 19, 2026 did not include orders for the routine CPAP cleaning described in the facility policy, and the clinical record did not contain documentation of weekly machine and humidifier cleaning or daily mask cleaning. An LPN stated that night shift managed the CPAP machines and confirmed the mask should be in a bag when not in use. Facility RN statements noted the mask was found on the nightstand without a bag on room rounds and that a bag was obtained, while the Regional Director of Clinical Operations confirmed the cleaning should have been ordered and implemented as per facility policy.
