Delayed Response to Blocked Urinary Catheter

Summary

The facility failed to provide nursing services consistent with professional standards of practice when an LPN did not timely monitor, recognize, communicate, or intervene for a resident with a blocked indwelling urinary catheter. The resident had diagnoses including urinary retention and benign prostatic hyperplasia with lower urinary tract symptoms, was cognitively intact, and had a care plan for a coude catheter. Physician orders directed staff to flush the catheter with 60 cc normal saline as needed for blockage or leakage and to change the catheter and drainage bag as needed. Nursing documentation showed the resident complained of penile pressure and abdominal discomfort, had zero urine output, a distended and firm abdomen, and grimacing during assessment. The resident stated he had told night shift about the problem and was told nothing could be done until day shift arrived. Documentation also showed attempts to flush the catheter were unsuccessful because of blockage. Later that morning, a newly inserted catheter drained 900 cc of yellow urine, and the resident reported total relief after the procedure. During interviews, the resident stated he reported severe abdominal pain and penile pressure after midnight and was told staff did not do that on the night shift and he would need to wait for day shift. The day shift LPN stated the night shift LPN gave only limited report that the resident's bed was wet, and that the night shift LPN did not provide additional information when asked whether evaluation or intervention had been attempted. The DON stated staff were expected to monitor catheter output each shift, evaluate suspected obstruction, attempt flushing, and obtain RN supervisor assistance if the LPN was not comfortable managing the coude catheter. The DON confirmed the night shift LPN was not comfortable changing the coude catheter due to lack of familiarity and waited until day shift arrived rather than notifying the RN supervisor or obtaining assistance.
