Summary

The facility failed to ensure two residents received necessary treatment and services consistent with professional standards of practice to promote healing of existing pressure injuries. The report cited facility policy requiring wound care to be prepared and completed using specified supplies and steps, including clean technique, dressing changes as ordered, and marking dressings with initials, time, and date. The deficiency was identified through clinical record review, observations, and staff interviews. One resident was admitted with diagnoses including a pressure ulcer of the buttock and disorientation, and the admission MDS showed the resident was cognitively intact with a BIMS score of 15 and had three Stage 3 pressure ulcers present on admission plus one unstageable pressure injury. A physician order directed cleansing the right buttock wound with 1/4 strength Dakin's solution, applying calcium alginate topped with Medi-honey, packing the wound with Dakin's soaked gauze, and covering it with a dry dressing daily and as needed. During an observation, the resident had no dry dressing in place, the right buttock was open to air in a soiled brief, serosanguineous drainage was present on the sheets, and no gauze packing was present in the wound as ordered. The LPN stated the resident should have had a dry dressing in place and then provided wound care as ordered. A second resident was admitted with muscle weakness and multiple wounds, and the admission MDS showed the resident was cognitively intact with a BIMS score of 15 and had one Stage 4 pressure area present on admission measuring 5.5 cm x 8 cm x 1 cm. A physician order directed cleansing the sacral wound with normal saline, applying collagen, then saline wet-to-dry gauze, followed by a foam dressing every day during the day shift, with changes as needed if soiled or dislodged. During observation, the coccyx dressing was in place but visibly soiled and had no date, time, or initials written on it, so it could not be determined when it had been applied. The findings were reviewed with the Nursing Home Administrator during interview.
