The HR Mgr stated the facility had no documentation in the files or computer showing when the last training occurred, and the NHA said he could not find the records in the previous DON's office.

Review of the facility's Continuing Education policy and Training Requirements-Required In-service Training for Nurse Aides policy showed that compliance with training requirements was a condition of employment and that nurse aide in-service training was to be sufficient to ensure continuing competence and no less than 12 hours per year. Review of the personnel records for NA Employee E2 and NA Employee E3 showed hire dates of 3/12/24 and 4/5/24, respectively, and their files did not contain documentation showing they had received the required minimum 12 hours of in-service training during their respective yearly periods. During an interview, Human Resource Employee E9 confirmed that the facility failed to ensure these two nurse aides received the required yearly in-service education.

During a complaint survey, documentation review of 5 of 5 Geriatric Nursing Assistant personnel files showed no evidence that the required annual 12-hour training had been completed within the past 12 months for GNA #25, GNA #26, GNA #27, and GNA #29. GNA #28's file contained documentation that the 12-hour yearly training was completed on 5/20/24, but there was no documentation that the training had been completed again since 2024. During the review on 5/15/26, the Human Resources Manager stated there was no documentation in the personnel files or computer showing when the last 12-hour training occurred and said there could be documentation in the previous DON's office, but was not sure. The Nursing Home Administrator later stated he could not find documentation in the previous DON's office related to any 12-hour training for the GNAs.

CNA Annual Abuse Prevention and DOJ Training Records Were Not Current

The facility failed to ensure that two of nine sampled CNA personnel records were current for annual Resident Abuse Prevention and Department of Justice (DOJ) training. During a concurrent interview and record review with the Director of Staff Development, the CNA personnel files were reviewed for completion of the annual Abuse Prevention and DOJ trainings. The review showed that CNA 1, hired 11/1/23, had no Abuse Prevention and DOJ training for 2025, and CNA 4, hired 9/4/24, also had no Abuse Prevention and DOJ training for 2025. The remaining sampled CNA records reviewed were documented as current or, for terminated employees, current at the time of review. During the interview, the Director of Staff Development confirmed that Abuse Prevention and DOJ trainings are conducted annually and that employees are expected to attend the trainings each year. The facility policy titled Abuse Prevention Program, revised 12/16, stated that the facility develops and implements policies and procedures to prevent abuse, neglect, or mistreatment of residents and requires staff training and orientation programs that include abuse prevention, identification, and reporting of abuse.
