Summary

The facility failed to store food items in a safe and sanitary manner and failed to maintain kitchen equipment in a sanitary condition in the main kitchen. During an initial tour with the Dietary Manager, surveyors observed a package of waffles in a freezer with no evidence of a date showing when they were placed there, opened, or needed to be used by. A package of frozen fish had a smudged and unreadable written date. An open box of salt stored with spices was not dated with an open date, and the shelf surface had a granule-like substance on it. Surveyors also observed four packaged rolls of ground beef thawing in a stainless-steel pan on the bottom shelf of a refrigerator next to four one-gallon plastic containers of milk, with one milk container in direct contact with the pan holding the meat. A black bag covering a mixer had dust, hair, and food particles on it. The hood over the stove had dust on the inner vents and on the fire suppression nozzles. The three-compartment sink was filled with various cookware to be washed, sanitizer strips used for testing had expired in July 2022, an empty food tray cart identified as clean had dried stains on the underside of the tray supports, and the curtain above the sink and the top of the window frame had stains, discoloration, and a significant build-up of dust.
