Summary

The facility failed to maintain a fully functional call light system for a semi-private room occupied by two residents, Resident 74 and Resident 127. Resident 74 was admitted with diagnoses including diabetes mellitus and kidney failure, had a BIMS score of 15 indicating intact cognition, and had severely impaired vision. Resident 74 stated the visual call light notification outside the room did not illuminate when the call system was activated, that the malfunction had existed since admission, and that staff were aware the system was not working properly. Resident 74 also reported delayed staff response at times and said he had to go into the hallway to verbally call for assistance, including after his roommate fell attempting to get back into bed. Resident 127 was admitted with diagnoses of diabetes and chronic kidney disease and also had a BIMS score of 15. Resident 127 stated the visual call light notification outside the room did not illuminate when the call light was activated and had been malfunctioning since admission. He reported that on May 4, 2026, he pushed the call light button while seated in his wheelchair and, after no staff responded, attempted to transfer himself back into bed without assistance and slid from the wheelchair to the floor. Facility investigative documentation confirmed he was found on the floor after attempting the transfer. Observation of the room showed the call light activation did not illuminate the visual notification outside the room, and no alternative method was in place for the residents to summon staff assistance. The Nursing Home Administrator was unable to provide evidence that the malfunctioning system had been timely repaired or that an alternative method had been implemented.
