Hand Hygiene Not Performed During Medication Administration

Summary

Failure to provide and implement an infection prevention and control program occurred during medication administration for one resident. Facility policy for hand hygiene stated that hand hygiene is to be performed before resident care and after contact with the resident's environment, and the medication administration policy stated that hands are to be washed with soap and water and gloves applied before administering topical, ophthalmic, otic, parenteral, enteral, rectal, and vaginal medications, with hand hygiene performed again after resident contact. During observation, a registered nurse prepared medications for the resident without using hand sanitizer or washing hands after disposing of the previous resident's medication cup. The same nurse was observed pouring Tylenol tablets from the stock bottle onto her bare hand and then placing the tablet into the medication cup, and she removed six additional pills onto her bare hand for the resident. No hand hygiene was performed during medication preparation. After administering the medications, the nurse again failed to perform hand hygiene. The Nursing Home Administrator and Director of Nursing later confirmed they would expect nursing staff to perform hand hygiene appropriately and not place pills onto bare hands before placing them in the medication cup.
