Elopement Risk Not Controlled and Exit Door Failed to Secure

Summary

The facility failed to implement interventions, supervision, and effective safety measures to prevent the elopement of a resident who had been identified as a high wander risk. The resident was admitted with diagnoses including aphasia, bipolar disorder, hemiplegia, and hemiparesis affecting the right dominant side. Two Elopement/Wander Risk Evaluations completed in February 2026 scored the resident as high risk, and the care plan identified the resident as an elopement risk/wanderer with an intervention that all staff, including the receptionist, should be aware of the resident’s tendency to wander. Despite that identification, the resident was not listed in the elopement book at the receptionist desk when surveyed. Nursing documentation showed the resident was observed ambulating aimlessly and was placed on elopement watch, but the DON stated that elopement watch meant staff should monitor and ensure the resident was not exit-seeking and also stated she did not believe the watch was documented anywhere in the clinical record. Additional notes described the resident as dressed and stating she was trying to go home, and later as confused, fully dressed, and repeatedly exit-seeking with packed belongings, a purse, shoes, and a jacket. On May 3, 2026, the resident was found outside the facility in the parking lot and near the dumpsters after exiting through a side door used for trash removal. Staff statements indicated the resident had last been seen inside earlier in the afternoon, and one employee did not know the resident was outside. The facility incident report stated the door was locked with a code but the locking mechanism was not fully engaged. Surveyors later observed that the same door could be pushed open without entering the code and that when it closed, the lock did not engage unless it was physically pulled shut. The DON confirmed the resident exited through that side door and that the door was able to be opened without the code.
