Summary

The facility failed to ensure that Resident 379 received necessary treatment and services consistent with professional standards of practice. Resident 379 had diagnoses including acquired absence of eye, schizophrenia, and dysphagia, and had a physician order beginning May 8, 2026, to cleanse the right eyelid with NSS, pat dry, cover with 4x4 gauze, and secure with tape every day and as needed on night shift. The facility policy for wound care required verification of the physician’s order and documentation of how the resident tolerated the procedure, any refusal, and other information in accordance with facility policy and professional standards of practice. Observations on May 11, May 12, and May 13, 2026, showed Resident 379 did not have gauze covering the right eyelid. The resident stated the gauze was not over the eyelid because it does not stay in place. The May Treatment Administration Record showed the gauze treatment was signed as completed on May 10, 11, and 12, 2026, on night shift. An RN stated the gauze should be in place at all times and there was nothing in nursing report indicating it was not in place. An LPN stated the resident had behaviors of self-removing the gauze but this had not yet been noted in the clinical record. The behaviors task did not note any behaviors since admission, and the care plan did not address refusal of treatment or self-removal of the gauze. The DON was informed of the concern, and no further information was provided.
