The facility failed to ensure a medication error rate of less than five percent during medication administration for 2 of 10 sampled residents reviewed for medication administration, resulting in an overall medication administration error rate of 11.11%. The deficiency involved insulin administration for two residents with diabetes mellitus. The report states that the 2016 insulin aspart (NovoLog) FlexPen manufacturer guide indicated the insulin pens should be primed with two units of insulin prior to each administration to ensure the correct dose was delivered. Resident 74 was admitted with type 2 diabetes mellitus and had physician orders in 6/2024 for insulin aspart 100 unit/ml, 7 units with meals. On 4/21/26 at 7:08 AM, Staff 31 (LPN) was observed administering insulin aspart to Resident 74 without priming the insulin pen prior to administration, and Staff 31 acknowledged this was not part of their practice. Resident 82 was admitted with type 1 diabetes mellitus and had 4/2026 physician orders for insulin aspart 100 unit/ml, 3 units with meals and insulin aspart 100 unit/ml per sliding scale before meals and at bedtime. On 4/21/26 at 7:21 AM, Resident 82 was observed leaving the dining room after already eating breakfast, and Staff 31 administered 6 units of insulin aspart after the meal; Staff 31 acknowledged the scheduled and sliding scale insulin were given after the resident had eaten. The DNS later stated insulin was expected to be administered before or with meals as ordered.
