Summary

Food items were not properly sealed, dated, or labeled in the kitchen refrigerator and storage areas. On 03/31/26, kitchen utensils were observed hanging over the sink where dirty dishes were washed. In the refrigerator, surveyors observed a bag of square brown patties with no label or date, a pitcher of orange liquid with no label or date, a bag of chunks of meat with no label or date, a bag of tan circular patties in an unsealed plastic bag with no label or date, and a cookie sheet of seven sandwiches dated 03/30/26 on the saran wrap, but the wrap was not sealed around the edge of the pan. Three cans of green beans were also observed without dates, and a plastic bottle of salad dressing was observed bulging on the top and bottom and not sitting upright on the shelf. The dietary manager stated the brown patties were fish, the pitcher was orange juice from breakfast, the round patties were cookies, and the other bag was fajita meat, and stated they knew they were supposed to label and date items. The facility also failed to keep food preparation areas clean and free from pests. On 03/31/26, four cockroach carcasses were observed in two drawers beneath the counter top where the blender was stored and used to puree foods for residents. The microwave next to the blender had food debris on all inside surfaces. Dietary staff stated the utensils hanging above the sink were used often and washed often, and a staff member stated the microwave was cleaned every day but residents used it often. The cook stated the drawers beneath the serving counter were not used because the dining room had not been served in over a year and did not know whose job it was to keep the drawers clean.
