Summary

Medication administration was not accurately documented in the medical record for Resident #26, who was admitted with diagnoses including weakness and hypertension and had mildly impaired cognition and anticoagulant use noted on the MDS. The resident had an order for Eliquis 10 mg twice daily through 01/14/26, then 5 mg twice daily beginning 01/15/26, with no discontinue date documented. A pharmacy receipt showed the facility received an 80-count supply of Eliquis 5 mg tablets on 01/09/26. The MAR showed the resident received the medication as ordered in January, with one refusal, and then continued to be documented as receiving Eliquis in February and March. Review of the medication counts showed the tablets should have been exhausted by 02/13/26, yet the MAR for February and March continued to indicate uninterrupted administration. The record contained no nurse progress note for 02/18/26 related to Eliquis administration, no documentation that the medication was out of stock or needed to be reordered, and no additional Eliquis was ordered in February. The administrator confirmed that if the medication ran out on 02/13/26, the February and March MARs were filled out inaccurately because the resident could not have received the medication as documented.
