Controlled Medication Given Without Active Order in Record

Summary

Nursing staff failed to verify and maintain a valid practitioner order in the resident’s clinical record before administering clonazepam, a controlled psychotropic medication, to one resident. The resident had diagnoses including hemiplegia and hemiparesis following cerebral infarction affecting the left non-dominant side, schizophrenia, bilateral above-the-knee amputations, anxiety disorder, and major depressive disorder. The only clonazepam order found in the medical record was for 0.5 mg twice daily as needed with a start date of 09/22/25 and an end date of 12/20/25. Review of the controlled drug sheets showed clonazepam was documented as given repeatedly from 01/02/26 through 04/06/26, but there were no corresponding MAR entries documenting administration of the medication. The narcotic log also showed no doses documented as administered for February 2026. Progress notes for the listed dates did not document that the resident requested clonazepam, complained of anxiety, or had behaviors recorded on those dates, and there was no documentation in the progress notes that the resident received clonazepam during February 2026. A DON progress note stated that the order was active with an active script the entire time of administration, but it was not active on the EMAR. During interview, the DON stated the medication had been renewed by the ViaQuest NP, but the nurse who received the verbal order did not enter it into the medical record or MAR. The DON verified the medication was administered without an active order in the medical record during the entire period reviewed. Staff interviews reflected that nurses expected medications to be on the MAR and would check for an order before giving a medication, but the record for this resident did not contain the order at the time the clonazepam was administered.
