Incomplete Infection Surveillance and Missing TB Screening Records

Summary

Provide and implement an infection prevention and control program was deficient because the facility did not maintain comprehensive infection surveillance and did not ensure staff had completed required TB screening. Review of the infection control log for January, February, and March 2026 showed it listed resident name, room number, date, type of infection, antibiotic, and whether the infection was facility acquired, but it did not include the type of bacteria, whether symptoms were present, or whether infections met McGeer's Criteria. It was also unclear whether the date reflected infection onset or the date antibiotics were started. The log included residents who received antibiotics for respiratory infection, post-surgery, UTI, tooth infection, skin infection, and pneumonia, including residents who were admitted and then treated with ciprofloxacin, Keflex, Macrobid, azithromycin, amoxicillin, doxycycline, and other antibiotics. For March 2026, the facility tracked residents receiving antibiotics and noted nosocomial or community acquired infections, but McGeer's criteria was not included on the log and the separate McGeer's form provided by the facility did not match current criteria. Several residents listed as receiving antibiotic treatment for infections were identified as not meeting McGeer's criteria, including residents treated for unknown infection, pneumonia, post-surgery, sepsis, wound infection, and UTI. In addition, review of personnel files showed no evidence of an initial two-step Mantoux for one CNA and no evidence of annual TB screening for multiple staff members, including housekeepers, a CNA, and an LPN. The Administrator stated the DON was responsible for maintaining evidence of staff initial and annual TB screening, but the DON was unavailable and the evidence could not be found.
