Summary

The facility failed to ensure that call lights were within reach for three residents out of 25 reviewed. Resident #22 had diagnoses including cerebral infarction, seizures, epilepsy, hemiplegia, and dementia, and the MDS showed severe cognitive impairment with assistance needed for multiple ADLs. The care plan identified that the resident was at risk for ADL self-care deficits and falls, and included that the call light should be in reach while in the room. During observation, the resident’s call light was attached to a box on the wall next to the bedside table light above the bed, approximately four feet away, and the resident was unable to reach it. An LPN verified that the resident could not reach the call light. Resident #39 had diagnoses including asthma, anorexia, major depressive disorder, and dementia, with the MDS showing moderate cognitive impairment and dependence or assistance needed for several ADLs. The care plan identified risk for ADL self-care performance deficit related to dementia with behaviors, seizures, gout, arthritis, anxiety, and delusions. During observation, the resident was in bed and the call light was wrapped around the left upper wheel under the bed, and the LPN verified it could not be reached. Resident #84 had diagnoses including diabetes mellitus type 2, anxiety, and major depression, with the MDS showing moderate cognitive impairment and need for assistance with toileting and personal hygiene. The care plan stated the resident was at risk for ADL self-care performance deficit and required assistance for toileting, transfers, and personal hygiene. During observation, the resident was lying in bed and the call light was wrapped under the left top wheel under the bed; an LPN verified the resident could not reach it. The facility policy titled Answering the Call Light stated the purpose was to respond to residents' requests and needs.
