The record showed physician visits on 04/24/25, 08/07/25, 09/11/25, 09/25/25, and 03/05/26, and the DON confirmed there were no other visits and that the resident was not seen by the physician as required between 09/25/25 and 03/05/26. Resident #44, with dementia with behavioral disturbance, psychosis, anxiety, type 2 diabetes, and schizophrenia, had moderate cognitive impairment with inattention and disorganized thinking; physician visits were documented on 07/03/25, 07/10/25, 08/21/25, 02/19/26, and 03/19/26, and the DON confirmed there were no other visits and that the resident was not seen as required between 08/21/25 and 02/19/26. Resident #53, admitted with Alzheimer’s disease, dementia, depression, anxiety, and Crohn’s disease, had severe cognitive impairment and was seen by the physician only once, on 09/04/25; the DON confirmed there were no other visits and that the resident had not been seen as required since that date. Resident #5, with multiple sclerosis, COPD, dementia, and chronic kidney disease, had severe cognitive impairment, rejected care four to six days in the review period, and was dependent for most ADLs; the record showed physician visits on 09/11/25 and 01/22/26, and the DON stated there were no other physician visits, only the nurse practitioner. Resident #35, with schizoaffective disorder, dementia, anxiety, hypertension, and COPD, had impaired cognition with moderate depression and fluctuating disorganized thinking; physician visits were documented on 07/10/25 and 11/20/25, and the DON verified the resident had not seen the physician since 11/20/25, only the nurse practitioner.

The MDS nurse and MRD could not locate PPN for the last three months, and the facility’s policy required the attending MD to write, sign, and date PPN at each visit and complete required resident visits on schedule.

Physician visit alternation was not documented for multiple residents. Records for residents with conditions such as COPD, dementia, diabetes, ESRD, Parkinson's disease, and HTN showed progress notes signed by the NP without evidence that required visits alternated between the physician and NP/PA as required by policy. For one resident, notes were signed by both the physician and NP but did not clearly show who actually saw the resident. The DON confirmed the missing documentation during interview.

Both Resident #7 and Resident #8 stated they did not believe they had been seen by a medical doctor in the facility.

The facility’s policy on Physician Visits stated the attending physician must visit residents at least once every thirty days for the first ninety days after admission and then at least every sixty days thereafter, and must complete relevant documentation at each visit. The facility’s policy on Physician Progress Notes stated the attending physician must write, sign, and date the physician progress notes upon each visit.

The DON and administrator stated the physician was responsible for seeing residents as required by regulation.

The report also identified deficiencies in the records of Resident R11, whose notes were signed by both the physician and nurse practitioner but were not clear as to who actually saw the resident; Resident R12, admitted 12/24/25 with Parkinson's, drug induced retention of urine, and hyperlipidemia; Resident R13, admitted 3/13/24 with Alzheimer's disease, anxiety, and hyperlipidemia; Resident R14, admitted 11/27/23 with muscle wasting and atrophy, weakness, and hypertension; Resident R39, admitted 1/9/25 with anxiety, insomnia, and hypertension; Resident R42, admitted 4/26/24 with COPD, dementia, and anxiety; Resident R55, admitted 3/10/25 with hypertension, emphysema, and hyperlipidemia; Resident R58, admitted 9/11/24 with Alzheimer's disease, hypertension, and chronic kidney disease; Resident R60, admitted 10/2/1018 with Alzheimer's disease, diabetes, and dementia; Resident R61, admitted 1/23/25 with hypertension, atrial fibrillation, and major depressive disorder; and Resident R94, admitted 10/6/21 with COPD, sleep apnea, and hypertension. During interview, the DON confirmed that the listed residents' records lacked evidence of alternating visits as required and that Resident R11's record did not clearly show physician visits between 6/26/25 and 1/16/26.
